lib: Reclaim aged leaked ssm pool blocks

The reclaim_pid_from_sc() function reaped any block allocated with refcount from a dead PID, but cross-process hand-offs can leave a block briefly allocated by the producer while a live consumer still holds it. lib: Fix SSM PUP creation on OS X

OS X doesn't support chmod on shm files after creation. Since we already set the mode at creation, that call was redundant. Fixed the getpeereid() function was not accessible because of the guards.
